In the case above, the mesial of the left central is too far labial, so a back bend is placed in the labial bow to exert lingual force on just the mesial edge…leaving space in the bow to allow the distal to continue to advance and rotate to the labial by directing the distal of the spring to the distal of the crown.
Comparing Options
Let’s compare NightShift® with the other options available to the person who wants straight teeth. Brackets and wires - work best, but look worst. They are expensive, involved and knowledge intensive, but definitely the best solution for cases that require substantial correction beyond cosmetic. Because of the way they look, what they cost, the inability to remove them, the hassle and discomfort to wear them… few mild to moderate cases will choose braces and instead do nothing or seek an alternative. Until now, the only alternative most people have been made aware of are invisible aligner trays such as Invisalign®. Invisalign vs. NightShift ComparisonInvisalign is the largest player in the removable aligner market. It’s a good product…but like everything else, it has its pros and cons. With Invisalign, price is the biggest concern for most doctors and patients and because of the cost, there is a huge market segment that has gone unfulfilled. As you’ve seen, it’s even more problematic in tough economic times. Invisalign has attempted to address the price issue by offering a less expensive express treatment…which, with fewer aligners, cannot be as efficient as the standard option for most cases. Express is typically chosen for simpler cases in the mild to moderate category. These are exactly the types of cases where NightShift can be a more convenient and much more affordable choice. Likely more efficient as well. An Invisalign® express case will still cost you at least 3 TIMES more than NightShift. Invisalign is much better suited than NightShift for more complex cases and those with significant spacing issues in which single or multiple large diastemas require significant mesial or distal movement of the incisors and/or posterior teeth to correct the problem…but that’s not the market NightShift targets. NightShift is certainly capable of some diastema closure via lingual movement using the standard labial bow, a retractable labial bow or hooks and power chain. Custom interproximal springs and elastics can also be used to close space effectively depending on the case. NightShift is best suited to treat cases where buccal movement is the predominant requirement, though lingual movement is obtainable by closing the labial bow, placing back bends in the bow, laser welding springs to the buccal or via hooks, buttons and elastics depending upon the indication. Unraveling crowded arches buccally and de-rotating even significantly rotated teeth is what NightShift does best. Below is an example of substantial crowding addressed efficiently, and please forgive the photo quality.
Progress shown here is 3 months and 6 days with full-time wear and bi-weekly adjustments. The teeth were not stripped to obtain this result. There was space in the upper to allow the arch to be widened at the cuspids in the lower. Nighttime-only wear would take longer but still produce the same result. Most nighttime cases have been finishing in 6 – 9 months, even less for simpler cases like the one below. Numerous doctors have said Invisalign can have difficulty rotating teeth efficiently. NightShift does this very well…even cuspids. Here’s an example of NightShift efficiently rotating teeth.
Progress shown here is 1 month and 4 days with full-time wear. NightShift also does a great job treating most anterior and some posterior cross bites requiring mainly buccal movement… though lingual is often possible by laser welding springs to the buccal. The example below shows NightShift® treating a cuspid in cross bite.
Look how efficiently this cuspid moved in 5 weeks with full-time wear and two adjustments. Remember there are several esthetic options that can be chosen if full-time wear is desired and the case is properly indicated, such as this one where a steel labial bow wasn’t required to obtain the result. NightShift treats a vast majority of cases with a single appliance that requires adjustments, though most find them intuitive and not difficult. The adjustment videos which are provided on the training CD and in the training section of this website should set you on your way quickly. Though Invisalign does not require adjustments, many doctors have said it’s not always quite as simple as just changing aligners every couple of weeks. If things don't go just right, remakes (reboots) can be necessary… causing delays in turn-around and treatment time which are always a disappointment to the patient and additional chair time for you. Invisalign covers all of the teeth like a glove, with the same tight-fitting tray. With this design only minimal force or directional variation can be caused upon individual teeth concurrently… unlike NightShift, which allows for substantial variation in force and direction with springs on each individual tooth. Because of the design and force delivery system, you’ll likely find stripping will be required more frequently with Invisalign than is necessary to get the same result with NightShift. The NightShift springs also deliver forces which create a very favorable alveolar bone remodeling response which you can compare for yourself. You can also add expansion screws, habit deterrents and more to the NightShift appliance, which isn’t an option with Invisalign. So…even though clear plastic aligner trays are a great concept that works well most of the time, they’re far more expensive and are required to be worn all day and night. Millions go untreated who would choose an option that could be worn only at night…or even full-time…if it could be done esthetically, efficiently and much more economically. Even if you love clear plastic aligners, you should add NightShift to your arsenal to reach a very large audience of people with indicated cases that won't move forward otherwise. The “Bulldozer” Appliance: in this category you will find appliances, which typically use an acrylic plate attached to coiled springs or a single “mushroom” type helix spring. They must also be worn full-time, which lessens their appeal. This type of appliance can't apply directional forces to a specific point on a crown… nor can it move teeth individually…which can make rotations and numerous other movements difficult or impossible. With this type of appliance you have no choice but to push on the most lingual malposed teeth first, even if they are partially blocked out of proper arch position by the adjacent teeth. For this reason, stripping is much more frequently necessary. Rather than push one tooth into another to bulldoze them into place, or strip away enamel, it’s much more efficient to first move the teeth which are closest to proper position into place, thereby providing space for the inhibited teeth to move freely. With individual springs on each tooth, NightShift allows you to most often times move teeth concurrently, providing uninhibited movement for each anterior tooth. Teeth can be moved sequentially by activating only indicated springs if concurrent movement isn’t feasible. The single plate or single spring appliances can be a good choice for really simple cases as some of them can treat such cases without adjusting… but it’s still not as attractive as part-time wear. Everything has its place. It’s up to you decide which tool is best for each job. Finger Spring Appliances: are far less than optimal for a number of reasons… primarily due to materials and design. Stainless steel is what most finger springs are made from and it deforms significantly and can't come close to delivering the consistent, optimized forces that NightShift® can… and you’re still stuck wearing them full-time. Sure… they're the least
expensive option, but they're utterly
incomparable to NightShift® in
convenience, efficiency and control.
Using them is generally penny wise and pound foolish
unless price is a very major issue, as there is not a large difference in cost. You can get a general overview of indications here and to a greater extent in the training materials.
